Chronic care refers to the management or ongoing treatment of long-term or pre-existing illnesses and conditions. 
Examples include:
- Coaching and support in areas such as weight management and nutrition 
- Physical and occupational therapy
- Non-urgent care for those in residential or long-term care facilities
