Cannabis use disorder has been found to be associated with an increased risk of both suicidal and nonsuicidal self-injury in veterans. This association remains significant even after adjusting for various factors such as sociodemographic characteristics, comorbidities, and past trauma. Studies have shown that current substance use disorders, including cannabis use disorder, are linked to a higher risk of suicide, with higher rates observed among both male and female veterans. Additionally, a review of international literature has indicated that chronic cannabis use, rather than acute use, is associated with suicidal thoughts and behavior.

11. Experience withdrawal symptoms, either:  A) Meeting the criteria for cannabis withdrawal syndrome (refer to Criteria A and B of the withdrawal criteria).  B) Using cannabis or a closely related substance to relieve or avoid withdrawal symptoms

Regular cannabis users often rely on the substance to cope with mood disturbances, insomnia, pain, or other issues, and individuals with cannabis use disorder often have concurrent mental disorders. Providers should be attentive to signs and symptoms of cannabis use and intoxication, such as red eyes, cannabis odor, chronic cough, and unusual food cravings. Chronic cannabis use can lead to a lack of motivation akin to persistent depressive disorder. It's important for healthcare professionals to assess cannabis use accurately, as individuals may underreport their usage.

 Associated Features of Cannabis use Disorder

Prevalence data reveals that cannabis use disorder affects both youth and adults, Within the cannabis user population, the prevalence of cannabis use disorder is approximately 20.4% among youth and 30.6% among adults. The 12-month prevalence of DSM-5 cannabis use disorder among adults is around 2.5%, with varying levels of severity. Despite an increasing number of individuals seeking treatment for cannabis-related problems, only a small percentage (7%-8%) of adults with cannabis use disorder receive cannabis-specific treatment, highlighting a significant treatment gap for this condition.

Cannabis use disorder commonly starts during adolescence or young adulthood but can occur at any age. The acceptability and availability of medical and recreational marijuana may influence its development, with increased onset among older adults. Cannabis use disorder typically progresses gradually over time, with increased frequency and quantity of cannabis use. Early onset of cannabis use, particularly before age 15, is a strong predictor of developing cannabis use disorder and other substance use and mental disorders in young adulthood. The perception of cannabis as less harmful than alcohol or tobacco has contributed to its increasing popularity among adolescents. The disorder is associated with risk-taking behaviors, conduct problems, and impairment in daily functioning. In adults, cannabis use disorder often involves daily cannabis use despite adverse consequences and unsuccessful attempts to quit. The rate of cannabis use among middle-aged and older adults is increasing, potentially influenced by availability and social acceptance, especially among the "baby boomer" generation.

Cannabis use disorder has wide-ranging consequences on various aspects of an individual's life. It can impair psychosocial functioning, cognitive abilities (especially executive functions), and overall health. These impairments can persist even when the individual is not intoxicated. The risk of accidents and compromised safety is increased. Cannabis use disorder is associated with a reduction in goal-directed activity and self-efficacy, leading to difficulties in school or work performance. It also has negative effects on social relationships, often resulting in problems within personal connections. Individuals with cannabis use disorder are more likely to experience reduced life satisfaction and require treatment or hospitalization for mental health issues.

 Cannabis Use Disorder Differential Diagnosis 

Cannabis use disorder is frequently associated with other substance use disorders, multiplying the risk for any other substance disorder by about nine. It is considered a gateway drug, as cannabis users have a higher probability of using more risky substances in the future. Co-occurring mental disorders, including major depressive disorder, bipolar disorder, anxiety disorders, and personality disorders, are common among individuals with cannabis use disorder. Cannabis use has been linked to an increased risk of psychosis, particularly when used during critical periods. Medical conditions such as cannabinoid hyperemesis syndrome, respiratory disorders, and adverse cardiovascular outcomes can also be associated with regular cannabis use.
