Research indicates that lower socioeconomic status is associated with a higher risk of health anxiety, with factors like stress, limited resources, and health-related behaviors helping explain this link. It also suggests that addressing social inequalities, such as by supporting disadvantaged groups, may be an important way to reduce health anxiety.

Illness Anxiety Disorder  (IAD) is a disorder, colloquially or previously referred to as hypochondria, where individuals experience stress and anxiousness about having an illness or pre-empting an illness that they have not been diagnosed with.  his can range from stressing about having certain symptoms to being keenly aware of any sensations that can link to the illness (however normal they may be) or investing time and effort trying to understand potential health concerns. Even when people do not have any illness and are told so by medical professionals, if they have IAD they are likely to continue to feel anxious.

Social inequalities in health anxiety

A person with IAD can experience a range of physical and psychological manifestations of the disorder. This can range from feeling anxious through the day about their health, and constantly checking biomarkers and vitals to an overreaction or extreme concern disproportionate to an actual diagnosis. In some severe cases, a person may also experience mild somatic symptoms like tachycardia, where the heart rate rises above normal. The combination of anxious, somatic, and actual symptoms can impede the lifestyle and well-being of a person experiencing IAD. 

Symptoms of IAD 

Care Seeking and Care Avoidant Type
Care Seeking Type: As the name suggests, people with this type of IAD tend to seek medical care and attention frequently, wanting to search for a diagnosis and medical explanation for their symptoms. This type also has a financial impact in terms of medical bills, check-ups and other procedures that the patient may invest in. They may also experience anxiety related to health insurance and their financial ability to be able to constantly check and address any illnesses. The health risk with this is that the medical professionals may not take some legitimate medical concerns seriously after a point. 

Care Avoidant Type: The person experiencing this type of IAD will be afraid to seek any medical attention regardless of symptoms, out of fear of the diagnosis that they may receive. While this may not have as many fiscal implications, it poses a different concern because the person still experiences anxiety and has no medical or professional intervention to invalidate or assuage their concerns. They may also have some legitimate symptoms or concerns that are not being addressed due to lack of medical care. 
 


Types of Illness Anxiety Disorder 

IAD and SSD can have a range of impacts on an individual’s quality of personal life and relationships. The constant fear and stress of potential illnesses may lead to a sense of anxiety and frustration for other close relations as well as an inability to focus on work and perhaps absences from work due to perceived illness. Even reassurance from medical professionals may only have a temporary effect and result in increased health care investments and fiscal responsibilities. Hence, when the symptoms of IAD and SSD affect persons functioning and well-being, it is important to seek treatment. The treatment is not for the various feared health conditions but for the underlying psychological disorder that drives this pattern of behavior. 
  


Impacts of Illness Anxiety Disorder and Somatic Symptom Disorder 

IAD and SSD may be lifelong but there is the possibility of treatment. The recommended treatment varies greatly and usually, a combination of approaches can be undertaken as there isn’t one concrete course of treatment that appears to be fully effective.  Rather than providing medication, lifestyle-based treatments are more recommended because they sustainably help address the cause and actively include the patient in the process of therapy. Given the nature of the illness, physicians play a very key role in balancing the line between validating the patient but also coaxing them into understanding that their concerns are not necessarily proportionate. It is also important to first gauge how receptive the person is to treatment and accepting their disorder in the first place. The downside of lifestyle approaches is that people are responsible for their own treatment so in cases where they are not fully receptive to the nature of their condition medicinal approaches might be more effective. Cognitive behavioral Therapy can also be effective in deconstructing and understanding the roots of IAD. Cognitive Behavioural Therapy may be sustainable in the long term enabling the patient to continue their own course of treatment. Alternatively, psychopharmacology is also suggested where patients are prescribed with SSRI’s.

IAD in Relation to Socioeconomic Status

There are two main existing theories regarding the development of IAD:
1. Cognitive-behavioral model (most supported): People misinterpret normal bodily sensations as serious illness → Leads to catastrophic thinking → Results in behaviors like excessive reassurance-seeking, symptom googling, and avoidance. Strongly supported by research.
2. Interpersonal model (less tested): Health anxiety stems from insecure attachment in early childhood → Individuals seek reassurance and care through physical symptom complaints. Aims to reduce underlying feelings of insecurity. Has less empirical support so far.

Development of Illness Anxiety Disorder

There is no consistent evidence of ethnic or migration‑related differences in health anxiety, likely due to limited research in these populations. However, research highlights that migrants and ethnic minorities may be especially vulnerable to health anxiety because of psychological stressors, social disadvantages, and barriers to good healthcare, and emphasizes the need for better research to understand these inequalities and inform appropriate support.

Health anxiety is common across the lifespan, especially in older adults, but is often underdiagnosed due to overlap with normal aging and physical health issues. Research emphasizes the need for age‑sensitive assessment tools and better research into how health anxiety presents and is treated in older populations.

Illness Anxiety Disorder in Older Adults

Research has found that health anxiety among young and middle‑aged employees was linked to higher levels of self‑compassion and more adaptive coping strategies, and these in turn were associated with greater participation in physical activity. It was also found that self‑compassion and coping strategies helped mediate the relationship between health anxiety and physical activity, suggesting that anxious workers may use exercise as a healthy way to manage their anxiety.
