- The study has a small sample size and short-term follow-up period, relief on self-report of substance use rather than biochemical verification, and included a heterogeneous sample of participants who used both alcohol and drugs.

- The clinic had pre-screened and excluded patients who had severe alcohol or drug problems.
- The intervention was not designed to discuss dependence.

- Patients in specialty psychiatric settings may have more severe depression problems than those in other contexts.
