In clinical settings, a psychiatric consult focused on the patient's relationships is used to determine if they have NPD. Countertransference can be helpful in reaching a diagnosis as the physician can see how the patient reacts emotionally to statements.

Method use to diagnose NPD in clinical settings

In research settings, to determine the severity of one's NPD, the five-factor narcissism inventory can be used. This is a questionnaire composed of 148 questions where one can rank statements from strongly disagree to strongly agree. Another narcissism test that can be used is the Narcissistic Personality Inventory.
