The study included 214 patients who had previously been infected with SARS-CoV-2. Clinical samples were collected at a mean of 44 days after the initial infection.

Participant Cohort for SARS-CoV-2 Serologic Assay Comparison

The 5 serological assays evaluated include the Euroimmun Anti-SARS-CoV-2 ELISA, the Epitope Diagnostics, Inc. (EDI) Novel Coronavirus COVID-19 IgG ELISA Kit, the ImmunoDiagnostics SARS-CoV-2 NP IgG ELISA kit, the Abbott-Architect SARS-CoV-2 IgG CMIA, and the Roche Diagnostics Elecsys® Anti-SARS-CoV-2 E-CLIA.

Serological Assays Evaluated for SARS-CoV-2 Antibody Detection

AUROC values (the area underneath the receiver operating curve) were calculated from an empirical receiver operating curve to compare the diagnostic performance of the 5 serological assays.

Statistical Comparison of SARS-CoV-2 Assays Using AUROC

To evaluate the performance of serological assays for SARS-CoV-2, 214 patients previously infected with the virus were tested using 5 different commercially available assays. The diagnostic accuracy of these assays was then compared using statistical measures.

Comparative performance of five commercially available serologic assays to detect antibodies to SARS-coV-2 and identify individuals with high neutralizing titers. 

All assays had the capability of testing for antibodies with AUROC values greater than 0.95. The sensitivity of the assay ranged from 76.4% to 93.9% and the specificity ranged from 87.0% to 99.6%. Abbot and Roche assays had higher specificity and higher diagnostic accuracy for Covid antibodies. The ELISA assays had a higher discriminatory capacity for detecting high nAbs. Whether the goal is to detect high nAbs or to detect the presence of protective antibodies should impact which assay is chosen.
