Selective serotonin reuptake inhibitors (SSRIs) are a class of drugs used to treat conditions like depression, which is often associated with low serotonin levels. Unlike agonists or antagonists that bind to receptors, SSRIs work by blocking the reuptake process. This action prevents unused serotonin from being reabsorbed by the presynaptic neuron, allowing it to remain in the synaptic cleft for a longer period. The extended presence of serotonin enhances its interaction with postsynaptic receptors, thereby strengthening its overall effect. Common examples of SSRIs include Prozac, Paxil, and Zoloft.

SSRIs and SNRIs

These older antidepressants can be helpful for alleviating cataplexy. Side effects include dry mouth and lightheadedness. 

Tricyclic antidepressants

This medication is very effective in treating cataplexy and improving nighttime sleep. In higher doses, it may also help with daytime sleepiness. It needs to be taken at bedtime and four hours later though, so it can be inconvenient in this sense. Side effects include nausea, bedwetting, and sleepwalking. Taking this with other sleep medications, narcotic pain relievers, or alcohol can be incredibly dangerous.

Traditional stimulant medications used to treat narcolepsy do not directly affect the hypocretin system, a key signaling pathway that is deficient in individuals with the disorder. Consequently, it is anticipated that new medications will be developed to specifically target and act upon the hypocretin system to provide more effective treatment.

Future of Narcolepsy Medication: Targeting the Hypocretin System

Psychomotor stimulant drugs, such as amphetamines, are a primary treatment for narcolepsy. These medications function by increasing overall levels of neural activity to combat daytime sleepiness. Examples include modafinil, armodafinil, and various amphetamines.

Stimulants for Narcolepsy

McCarty's (2010) case study describes a 50-year-old woman who was diagnosed with narcolepsy after sleep testing. She had experienced excessive sleepiness during normal waking hours for several years and had fallen asleep in inappropriate or dangerous situations, including while eating, socializing, and driving. During emotional arousal, she experienced unusual right-sided cataplexy but did not have dream-like hallucinations. A stimulant alone was not successful, but combining the stimulant with an antidepressant led to dramatic improvement.

Case Study of Narcolepsy: Symptom Variability and Combination Therapy

A patient diagnosed with narcolepsy begins treatment with a standard psychomotor stimulant. After a month, they report that their excessive daytime sleepiness has only slightly improved, and they still experience frequent episodes of sudden muscle weakness triggered by laughter. Narcolepsy is a sleep disorder characterized by an uncontrollable urge to fall asleep at inopportune times. These sleep episodes are frequently accompanied by cataplexy, a sudden loss of muscle tone. Cataplexy is a sudden lack of muscle tone or muscle weakness, sometimes resulting in the complete paralysis of voluntary muscles. This condition is a common symptom of narcolepsy, analogous to the muscle atonia experienced during REM sleep. Approximately one-third of individuals with narcolepsy experience vivid, dream-like hallucinations during their sleep attacks. These hallucinations are another feature that narcoleptic episodes share with REM sleep.

Hallucinations in Narcolepsy

Narcoleptic episodes are often paradoxically triggered by heightened emotional states, such as arousal or stress. These sleep attacks typically last for a brief period, ranging from a few minutes to half an hour. Following an episode, individuals commonly report feeling refreshed.

Characteristics of Narcoleptic Episodes

The recurrent and uncontrollable nature of narcoleptic episodes can significantly disrupt daily life. These disruptions often impair performance in academic and professional settings. Furthermore, narcolepsy poses serious safety risks, as episodes occurring during activities like driving or operating heavy machinery can lead to severe harm or injury.

Consequences of Narcolepsy

Narcolepsy is strongly associated with reduced levels of the signaling molecule hypocretin, also known as orexin, in certain brain regions. These neuropeptides are produced in the hypothalamus and are crucial for regulating appetite and the transitions between sleep and wake states. The degeneration of hypocretin-producing neurons is thought to cause the improper activation of the cataplexy pathway, which is normally active only during REM sleep. The loss of these neurons, which project to key arousal centers in the brain, leads to the abrupt and uncontrolled transitions between wakefulness and sleep characteristic of narcolepsy.

Role of Hypocretin in Narcolepsy

A patient reports experiencing overwhelming daytime sleepiness, causing them to fall asleep unexpectedly during meetings. They also describe episodes where strong emotions, like laughter, trigger a sudden, temporary loss of muscle control, causing them to slump over. Additionally, they mention having vivid, dream-like visions just as they are falling asleep. Based on this specific combination of symptoms, which of the following is the most likely condition?

Variability in Narcolepsy Symptoms and Treatment Response

In general, Mindfulness Based Interventions have the strongest effects on self-reported sleep quality and on decreasing total wake time in people with insomnia. Additionally, it has been shown to positively affect the severity of insomnia as well. These benefits are achieved because through the the targeting of arousal in mindfulness practices, a mindful stance can be achieved, thus decreasing sleep arousal and allowing for normal sleep patterns to emerge. 

Mindfulness and Effects on Insomnia

Cognitive Behavior Therapy for Insomnia is the most widely used and recommended psychological treatment for chronic insomnia. CBT-I uses interventions involving sleep hygiene, stimulus control, sleep restriction, and cognitive therapy. CBT-I is moderately effective, but may not address hyper-arousal, which is known as a perpetuating factor for insomnia. If CBT-I is not effective in reducing hyper-arousal, then Mindfulness-Based Therapy should be used as treatment for chronic insomnia. MBTI uses the development of a metacognitive process and meditation practice to reduce cognitive and physiological hyper-arousal.Through the improvement of maladaptive thinking instead of challenging content directly, MBTI is ideal to improve sleep for those who do not respond to CBT-I.
