The Diagnostic and Statistical Manual of Mental Disorders (DSM) has undergone significant transformations since its initial publication. Early editions controversially classified homosexuality as a mental disorder, a position that was officially reversed in 1973. Subsequent editions, particularly from DSM-III onwards, featured a substantial increase in the level of detail for describing disorders. This led to a growth in both the number of diagnosable conditions and the physical size of the manual, a trend that was later reversed with the DSM-5 containing fewer disorders than its predecessor. More recent versions also reflect a greater emphasis on the influence of cultural and gender contexts on the presentation of symptoms.

The History and Evolution of the DSM

In 1917, the Statistical Manual for the Use of Institutions for the Insane was published by the American Medico-Psychological Association (AMPA) in collaboration with the National Commission on Mental Hygiene (now Mental Health America). It was the first standardized classification manual and described 22 major diagnostic categories along with their most common manifestations. It mostly focused on the biological aspects of mental disorders. There would be 10 editions of the manual up to 1942 at which it was titled the Statistical Manual for the Use of Hospitals of Mental Diseases. 

Statistical Manual for the Use of Institutions for the Insane

In 1933, the Standard Classified Nomenclature of Disease (known simply as Standard) was published by the American Medical Association (AMA) along with the New York Academy of Medicine. It was notable for its inclusion of a nomenclature of psychiatric conditions provided by the American Psychological Association (APA).

In 1945, shortly after WWII, issued in War Department Technical Bulletin, Medical 203, the Office of the Surgeon General released Nomenclature and Method of Recording Diagnoses. Influenced by soldiers’ experiences in WWII, this issue was designed in an attempt to move away from the traditional focus on mental institutions and clinical perspectives. In 1946, it was reprinted for public use in the Journal of Clinical Psychology under the title of Nomenclature of Psychiatric Disorders and Reactions (known as Medical 203).

In 1949, the World Health Organization (WHO) released the 6th version of the International Classification of Disease (ICD). This version was the first that included a section dedicated to mental disorders. This section was similarly revised from Medical 203 and lacked traditional medical standardization.

In 1968, the American Psychological Association (APA) published the DSM-II in an effort to revise the first edition in accordance with 1960s overall challenges to the concept of mental illness. Due to critiques and arguments from psychologist, sociologists, activists, and others, this version resulted in broad and vague interpretations of symptoms that resulted in a lack of consistency in diagnostic practices.

In 1974, the 7th printing of the DSM-II was published by the American Psychological Association (APA), with revisions made due to the success of gay rights activist, Frank Kameny and the Gay Liberation Front. This printing was prominent for its removal of Homosexuality as a category of disorder. However, the diagnosis was replaced with the category of Sexual Orientation Disturbance.

In 1987, a revision of the DSM-III was published under the continued direction of Robert Spitzer and the American Psychological Association (APA). This revision mostly focused on some recategorization of certain diagnoses, while editing or removing controversial ones.

In 1994, the DSM-IV was published by the American Psychological Association (APA) with guidance from a new taskforce headed by Allen Frances. The revisions for this edition were largely focused around providing more research credibility to the existing diagnoses, while also adding stricter “clinically significant” requirements for most symptoms to be considered for a diagnosis. This was in accordance with an updated definition of mental disorder, focusing on the same requirement of “clinically significant” aspects of distress or disability.

In 2000, a Text Revision (TR) of the DSM-IV was published by the American Psychological Association (APA) with minor revisions made and several updated codes to keep consistency with the  9th edition of the International Classification of Disease (ICD-9). Only 9 diagnoses were edited, while the rest of the text remained the same.

in 2022, a Text Revision (TR) of the DSM-5 was published by the American Psychological Association (APA) with only minor diagnostic changes and updated codes to match the 10th edition of the International Classification of Disease (ICD-10). The most prominent revision was the inclusion of sections for each diagnosis that discussed the influence of social contexts such as sex and gender, race and ethnicity, racial and cultural variations, and others.

The classification of homosexuality in the DSM has been a subject of significant change, reflecting evolving societal and professional perspectives. Initially, the first two editions of the DSM listed homosexuality as a mental disorder. Following a vote by the American Psychiatric Association in 1973, it was formally removed from the manual, marking a pivotal moment in the history of psychiatry and gay rights.

The number of diagnosable conditions included in the DSM has fluctuated significantly across editions. The DSM-I began with 106 diagnoses. This figure grew substantially to 265 in the DSM-III and peaked at 297 in the DSM-IV. In a notable reversal of this expansionist trend, the subsequent DSM-5 featured a reduced count of 237 disorders.
