Biblical Quarantine Practices

Constructed around $${}2500$$ BCE (approximately $${}4500$$ years ago), the ancient Indus Valley Civilization cities of Mohenjo-daro and Harappa featured some of the earliest known urban sanitation systems. These communities utilized complex networks of wells, baths, and drainage systems to supply fresh water and remove waste, establishing foundational practices for healthy societies.

To protect the population from epidemics of waterborne illnesses, ancient Romans engineered sophisticated sanitation infrastructure. They constructed aqueducts to deliver fresh water into the city and built the Cloaca Maxima, a massive sewer system that directed waste away from urban areas and into the river Tiber.

Ancient Roman Sanitation

The Greek physician Hippocrates ($${}460$$–$${}370$$ BC), often considered the “father of Western medicine,” rejected the idea that disease was caused by supernatural forces. Instead, he proposed that diseases arose from natural causes within patients or their environments. He and his successors are credited with the Hippocratic Corpus, which includes some of the oldest surviving medical texts. He is also associated with the Hippocratic Oath, a pledge taken by new physicians to diagnose and treat patients without doing harm.

Hippocrates and Early Western Medicine

The Greek philosopher and historian Thucydides ($${}460$$–$${}395$$ BC) is considered the father of scientific history due to his advocacy for evidence-based analysis and cause-and-effect reasoning. A survivor of the Athenian plague, which killed one-third of the population of Athens between $${}430$$ and $${}410$$ BC, he observed that survivors did not become re-infected even when caring for actively sick individuals. This significant observation demonstrates an early historical understanding of the concept of immunity.

Thucydides and the Concept of Immunity

Marcus Terentius Varro ($${}116$$–$${}27$$ BC) was a prolific Roman writer and one of the earliest individuals to suggest that invisible entities could cause disease. In his work Res Rusticae (On Farming), published in $${}36$$ BC, he warned against neighborhood swamps, proposing that unseen creatures, which he termed animalia minuta (minute creatures), floated in the air, entered the body through the mouth and nose, and caused serious illnesses.

Marcus Terentius Varro and Invisible Pathogens

In $${}1546$$, the Italian physician Girolamo Fracastoro proposed in his essay 'De Contagione et Contagiosis Morbis' that seed-like spores could be transferred between individuals through direct contact, exposure to contaminated clothing, or the air. He is now recognized as an early proponent of the germ theory of disease, which states that microbial infections cause diseases. However, his ideas were not widely accepted in the $${}16$$th century and were largely forgotten until the $${}19$$th century.

Girolamo Fracastoro

In the $${}11$$th century, the Persian physician Ibn Sina, sometimes referred to as Avicenna, proposed that tuberculosis was likely spread by people's breath when they were in close proximity to one another. His observations contributed early evidence to the understanding that unseen substances could cause diseases.

Writing around $${}1155$$, the Arab physician Ibn Zuhr documented that the common skin condition scabies was caused by tiny mites that bored into the skin. Although scabies mites are about half a millimeter long and not technically microscopic, his discovery provided early evidence that unseen substances or creatures caused diseases.

Prior to the discovery of microbes, the ancient Greeks proposed the miasma theory, which held that disease originated from particles emanating from decomposing matter, such as sewage or cesspits. These particles were believed to infect humans who were in close proximity to the rotting material. During the Middle Ages, diseases such as the Black Death were thought to have originated in this way.

Even prior to the invention of the microscope, various ancient civilizations recognized that diseases could be transmitted by invisible entities, which are now understood to be microbes. This early awareness of communicable diseases led to the implementation of public health measures, such as quarantining the sick and building comprehensive sanitation systems, to mitigate the spread of infections.
