Abnormalities in the circuitry of the prefrontal cortex can contribute to impairments in working memory, though in only a specific subsection of GABA neurons. Local circuit neurons regulating these types of functions in the prefrontal cortex show altered GABA neurotransmission. There was also reduced mRNA expression (below the level of detection) of glutamate decarboxylase (GAD₆₇), which is an enzyme responsible for making GABA in the prefrontal cortex, and GABA membrane transporter-1 (GAT-1) - which is seen in 25-35% of GABA neurons in the prefrontal corteces of those with schizophrenia.


Abnormal Prefrontal Cortex Circuitry & GABA Neurotransmission in Schizophrenia Patients

- Parvalbumin (PV) mRNA positive neurons were most highly distributed in prefrontal cortical layers II-IV. In schizophrenia patients, a 29% decrease in expression in layer III and a 35% decrease in layer IV was seen
- Calretinin (CR) mRNA positive neurons were most highly distributed in prefrontal cortical layers I and II. There was no significant difference in CR mRNA expression in these layers in patients with schizophrenia
- PV mRNA signals in area 9 of the prefrontal cortex are decreased in patients with schizophrenia but CR mRNA signals aren’t affected
- Average optical density of PV mRNA was decreased by 30% in patients with schizophrenia
- Grain density per PV mRNA-positive neuron showed a 22% decrease in patients with schizophrenia

Expression of parvalbumin and calretinin mRNAs in area 9 of schizophrenic and control subjects

Overall, there was a significant correlation between reduced neuronal PV mRNA expression and a decreased density of GAD₆₇ mRNA-positive neurons in those with schizophrenia. There was a strong correlation when comparing the within-pair differences in GAD₆₇ mRNA-positive neuron density and the in grain density per PV mRNA-positive neuron. There was no correlation in the differences of GAD₆₇ mRNA-positive neuron density and in PV mRNA-positive neuron density. There was also no significant correlation in the differences of GAD₆₇ mRNA-positive neurons and CR mRNA-positive neuron density or grain density per CR mRNA-positive neuron. GAD₆₇ mRNA signals were undetectable in many (about 45%) PV mRNA-positive grain clusters - the amount of these neurons with undetectable signals was 4.5 times greater in those with schizophrenia. These results suggest that GAD₆₇ mRNA expression is deficit in schizophrenia patients only in a selective subclass of PV-containing GABA neurons in the prefrontal cortex

Substantial evidence indicates a genetic foundation for schizophrenia. For instance, an individual's risk of developing the disorder is approximately six times higher if they have a parent with schizophrenia compared to someone who does not. Furthermore, the likelihood of developing schizophrenia is directly correlated with the degree of genetic similarity to family members who have been diagnosed with the condition.

Hereditary Component of Schizophrenia

Individuals with schizophrenia on average have lower overall physical health and are at increased risk of infection with COVID-19 due to decreased cognitive ability and decision making capacity. In addition, the psychotic symptoms associated with schizophrenia may be heightened due to social distancing and isolation practices. These practices can make basic services and support difficult to access for schizophrenic patients, making them more vulnerable to a negative impact on their mental health.

Treatment of Schizophrenia 

Studies have found that apart from the genetic factors that are responsible for schizophrenia, there is also a variety of stressful events that may increases the risk of developing schizophrenia. Some of them are:

Role of Environmental Factors in Schizophrenia 

There is evidence of increased neuroinflammation and inflammatory cytokines in the CSF of schizophrenia patients within 5 years of onset, as well as increased microglia density in the dorsolateral prefrontal cortex. Abnormal microglia density in the temporal lobe seems to be consistent across all schizophrenia patients, but there appears to be some nuanced differences among different subtypes and among the brains of patients who died by suicide versus not. Antipsychotic exposure seems to impact microglial density and inflammation, but this is not yet well understood.

Though there is no cure for schizophrenia, many patients do well with minimal symptoms. A variety of antipsychotic medications are effective in reducing the psychotic symptoms present in the acute phase of the illness, and they also help reduce the potential for future acute episodes and their severity. Psychological treatments such as cognitive behavioral therapy or supportive psychotherapy may reduce symptoms and enhance function, and other treatments are aimed at reducing stress, supporting employment or improving social skills.

Environmental Risk Factors of Schizophrenia

An uninterrupted illness featuring at some time a major depressive episode, manic episode, or mixed episode concurrently with characteristic symptoms of schizophrenia (e.g., delusions, hallucinations, disorganized speech, catatonic behavior) and, in the same period, delusions or hallucinations for at least 2 weeks in the absence of prominent mood symptoms. DSM–5 identifies the mood episodes only as either major depressive or manic and emphasizes that mood disturbances must be present for a majority of the time.

Reference for Downward Drift

Downward drift is the term given to the deterioration of the lives of individuals suffering from schizophrenia in which the disorder leads them to poverty, which obstructs their capability to work.

Downward Drift 

Unlike what the name suggest, schizophrenia does not mean split personality. Rather it is encompass symptoms such as dissociative thinking, emotional disturbance, delusions and hallucinations. These symptoms can be broadly divided into three categories:

- Positive Symptoms: behaviors that are present or gained

- Negative Symptoms: behaviors that are absent or lost

Symptoms of Schizophrenia

The severity, duration, and frequency of these symptoms can vary, and severe psychotic symptoms often decrease with age.

Schizophrenia affects approximately 1% of the population over a lifetime. The disorder is typically first diagnosed in early adulthood, most commonly in the early to mid-20s.

Individuals with schizophrenia often face substantial challenges in daily functioning, including difficulties with employment, financial management, personal hygiene, and maintaining social relationships.
