Case Management for Comorbid Psychotic Disorder and Substance Use Disorder
The goal of case management is to improve treatment adherence and facilitate successful reintegration through helping patients navigate the mental health and social services systems. This is particularly important for individuals with psychotic disorders, as they have relatively low treatment completion rates. Case management approaches typically involve five main components: an assessment of needs, development of a comprehensive treatment plan, outreach and ongoing coordination of services, monitoring, and evaluation and follow-up.
